Teaching Read Chapter 2 In this chapter you will read about the basic steps of Case Management: Assessment, Planning, Linking and Monitoring. It is important to remember that Case Management does not include therapy. Counseling the client is not part of the job. Instead the Case Manager works as a resource coordinator to empower the client to access the needed services. Sometimes the client needs multiple services. The Case Manager can help the client by letting them know of sources that provide multiple services. We need to keep the reality of implementation in mind as well. Assessment Lesson 4 Discussion As a case manager, it is crucial to know what local resources are available. Research local services, agencies, support groups. You can start with the phone book, look online or reach out to local people in the community. This collaborative exercise is designed as a starting point for a list of local resources. To support that collaboration, we will be working in a Blog format this week. In your Main Blog post, profile one local resource.
